The beginnings of organized trauma care can be traced back to military conflicts where emphasis on quick treatment prompted advances in field transport and mobile army surgical hospitals located near front lines. Major innovations in the treatment of injured soldiers were developed during WWII and further refined during the Korean War, Vietnam War, and the introduction of helicopters. This made possible rapid transfer of injured soldiers to the nearest available hospital with the resources to treat their injuries.

Dr. Jeff Claridge completed his medical degree at the University of Rochester and surgical residency at the University of Virginia. He is currently the Service Line and Division Director of Trauma, Burns, Critical Care and Acute Care Surgery at MetroHealth Medical Center in Cleveland, OH. He has extensive experience in ECMO and other extracorporeal life support (ELS) therapies.

He is the Surgical ICU Medical Director at the Ellison 4 SICU, Blake 12 Surgical ICU and the B8/E9 Heart Center ICU. He is an active member in multiple professional societies and has extensive research and education involving the traumatically injured patient.

Closing Gaps in Trauma Care: Steps Towards a National Trauma System

Trauma centers are staffed with doctors, nurses and medical experts who treat patients with injuries ranging from mild to severe. These patients are usually brought in through the emergency room.

The ACS COT has made great progress toward a national trauma system since 1922, but many gaps remain. A recent report from NASEM outlines steps to close those gaps.

Level I Trauma Centers

A level 1 trauma center has the most comprehensive treatment options and increases a person’s chances of survival with life-threatening injuries by 20 to 25 percent. It has a full range of specialists, equipment and facilities available 24-hours a day and admits a minimum annual volume of severely injured patients.

The ACS Committee on Trauma (COT) evaluates and verifies trauma centers based on their readiness, specialized trauma equipment, policies and patient care. This verification is valid for three years.

Pediatric trauma centers have the equipment and training to treat injured children from birth through age 18. Although only 7% of trauma centers are considered pediatric, this type of facility is important because injuries lead to more childhood deaths than any other cause. The UC Davis Medical Center, for example, has one of the only pediatric trauma centers in California and partners with community organizations to provide injury prevention programs, such as teddy bear clinics, senior falls prevention events and courses that teach children to stop bleeding.

Level II Trauma Centers

When a person suffers a traumatic injury, whether a blunt force injury (such as a broken bone) or penetrating wound, he or she must receive specialized medical care as quickly as possible for the best outcomes. Definitive Healthcare tracks 2,229 trauma centers in the United States through its HospitalView product.

Concord Hospital is one of these centers and has recently undergone verification by the ACS Committee on Trauma, a process that examines commitment, facilities, resources, training, prevention education, staffing, documentation, and the ability to treat severely injured patients around-the-clock.

The Level II Trauma Center designation demonstrates Concord Hospital’s capability to provide the necessary initial assessment and resuscitation of a severely injured patient prior to transfer to a higher level of trauma care. This is especially important for rural areas that do not have access to a Level I trauma center. It also allows a patient to bypass the longer, more expensive journey by ground ambulance to a nearby trauma center with the same resources.

Level III Trauma Centers

Trauma centers are designated hospitals equipped to provide specialized services and resources for injured patients. They play a critical role in saving lives each year by treating more than 275,000 people with traumatic injuries each year. Definitive Healthcare’s HospitalView product tracks 2,229 trauma centers in the United States.

These hospitals must be able to identify and triage trauma patients, and they must have surgeons on call 24 hours a day to handle any emergencies. They also must be able to stabilize the patient and arrange for transfer to a higher level of care.

Under 2022 standards, all adult Level II and pediatric Level II trauma centers will have to develop transfer protocols that specify “the type of patients/injuries that will be transferred to a center with an orthopedic trauma (OTA) fellowship trained orthopaedic surgeon” (Standard 4.12). The new standard also requires trauma registrars to complete training in abstraction, data validation and registry-related topics.

Level IV Trauma Centers

Trauma centers are hospitals equipped to provide specialized resources and services to treat severe injuries. They complement traditional hospital and emergency department care. More than 275,000 people die every year from traumatic injuries, making trauma centers essential.

The level of a trauma center is indicated by its designation level, with levels 1 through 5 (some states acknowledge five different levels). Higher-level trauma centers offer more comprehensive care than lower-level ones.

Level IV trauma centers are designed to resuscitate and stabilize patients prior to transfer to a higher-level center. They also have diagnostic capabilities.

Medical City Healthcare hospitals that are designated as a Level III or Level IV trauma center can provide Stop the Bleed training, a course in bleeding control created by the ACS for thousands of first responders, healthcare professionals and community members each year.

We assessed trends in nationwide access to American College of Surgeons Committee on Trauma-verified trauma centers between 2013 and 2019. We based our analysis on fastest travel time (by ground or air) from a census block group population centroid to the nearest level I, II or III center.
